Older adults with kidney disease face high morbidity and mortality rates, highlighting the importance of simplifying the ACP process. Advance care planning (ACP) has expanded to include care planning throughout life, not just end-of-life preferences. This invited commentary was designed to share helpful phrases and evidence-based resources like PrepareForYourCare with a nephrology audience and was published online on August 1st. It focuses on making ACP easier for adults with kidney disease and their clinicians by providing user-friendly tools.
